Defendant County of Sacramento's motion for summary judgment is granted. Plaintiffs are the mother and daughter of decedent Kelley Chamblee. On November 1, 2002 Chamblee came to the County's mental health center to be treated for depression and bipolar disorder. She was given a prescription for Geodoh and released the following day. Her prescription was for 30 days. She was referred to EL Hogar for out patient care. She died suddenly of cardiac arrest in October 2003. The forensic pathologist's report indicates the records showed she was taking Gepdon which can cause heart problems. Defendant has submitted the declarations of the two doctors who treated Chamblee at the County, Morache and Leamon. They declare there was nothing in her records that contraindicated Geodom and that it did not cause her death. Plaintiffs submit the declaration of Dr. Blinder. He declares that Geodonn has notable side effects over time and with dosage increase. He refers to an article in the American Journal of Psychiatry, April 2006 which sets forth the various precautions that must be taken before prescribing such drugs. The forensic pathologist, Fiore, declares in reply that he cannot state, and has no evidence, that Ggeodon was taken before death and that it played any role in her death. He also states that in his report he did not mean to infer that Geodon was a "probable cause" of death. The medical history of Chamblee shows that the County doctors prescribed Geodon. It was while Chamblee was receiving treatment at El Hogar that additional drugs were prescribed and Geodon was increased from 40 BID to 80 BID. A journal article published four years after the alleged malpractice of the County doctors cannot be used to establish the standard of care. Assuming the County doctors should have conducted a more extensive evaluation of Chamblee before.
Patients placed on an atypical in a State Hospital will be continued as is clinically appropriate after discharge. Patients transferring into the NorthSTAR service from a community mental health center who are on an atypical, will not be placed on the wait list, although the authorization will be contingent upon reviewing for appropriateness of medication usage. In the last 10 years, my diagnosis has been amended to schizoaffective disorder. There's definitely an affective component to my problems. Specifically, my diagnosis is schizoaffective bipolar II, with periods of hypomania and depression. Sometimes, there's rapid cycling between those two, and controlling the affective part of my diagnosis has been challenging. That's a real important part of how I stay functioning. The basic problem is schizophrenia. It's a cognitive problem, but the affective part is certainly there, and I take medication for both. I'm on a mood stabilizer and an antipsychotic. I'm taking Geodon, and I take Topamax for the affective part. Geodon has totally transformed my life. I've been on it about 6 years now, and before taking it, I had been hospitalized more than 100 times. I've stopped going into the hospital--it's not a part of my life. So it's had a profound impact. And of course, that's had a huge impact on the kind of art I'm doing. Over the last 6 years, my imagery has gotten a lot more light into it. I'm working now on a series on lotuses. For me, the lotus is the embodiment of the life force. In fact--this is a scientific fact-- if you hooked 40 lotuses to a certain kind of electrical apparatus, they would put out enough energy to illuminate a light bulb. They have that much energy. That's just the way they are, and they fascinate me. I was married and living in Santa Cruz with my husband, when we were transferred to Oregon in 1992. I live in a rural area now among Christmas tree farms. It's beautiful, but I'm divorced now. The illness took its toll on the marriage. I was married about 7 years. He knew about my illness beforehand, but he really didn't understand. He did not know really what it would entail. But we're still good friends. I don't have family here, but I have some very, very good friends. My family is in Georgia and Texas, which is where I'm from. picture about it as a way to make sense of a situation. This is the way that I take care of myself. There's never been a time of my life when I did not draw. I was 7 years old when Kennedy got shot, and I had to draw a picture of it. I'm 49 now. There's still plenty of turbulence around the edges of my life, but part of what I've been doing for the last [few] years is meditation. It's a big part of my life. I'm sure that it influences something in terms of serenity. It's helped me. I haven't been teaching lately, but I put in a proposal to teach a class this spring called "The Practice of Beauty." I'm excited about doing that. The class will combine mindfulness techniques with drawing. Before the Geodon clinical trials, I was going to the hospital every month and being put in seclusion against my will. It turned my life around. For the clinical trial I traveled down from Oregon to Los Angeles, first once a week for 8 weeks, and then once a month for 2 years. It was about a 14hour trip. I was very motivated. This is why when I'm painting, the imagery of the life force means a lot to me, because this life force is something that I feel deep inside me. Now I celebrate it in my art. The life force got me through the clinical trial, and it has refused to be extinguished. "Lotus With Sparks" is about light, energy and overcoming. It's feminine, but there's also a masculine element. With "White Lotus No. 10 ; , " I wish you could see this in person. There are little marks that are incised into the background. There's also a very light under-painting beneath the black background, and the incised marks allow the lighter background to show through. This is a drawing technique, and I liked being able to combine drawing techniques with painting techniques here. There is a luminous quality to this picture. A painful one is the "Hugging Form No. 1 ; ." That was done in 1997 right before the clinical trial. It exposes how I was feeling. I was putting down the way I felt in my body. I had a friend who wanted to buy it, but when she took it home her husband thought it was too disturbing, so she had to buy something else; something lighter. It's important to me to get my work out there. The feedback I'm getting is very positive, and I can't think of a better way to reach people. It's been a long time since I exhibited. I have shown some things through NARSAD [National Alliance for Research on Schizophrenia and Depression]. I have a pond outside my bedroom that I built in my yard, and I have koi and water lilies. It's a beautiful form in nature. I never would have thought of painting a lotus back when I was going to the hospital and so preoccupied with pain and death. I would have thought it was trivial. The ketone bodies found in urine during starvation could also be produced by a diet in which most of the calories came from fat vs a diet in which most of the calories came from carbohydrates and protein. Typically, a ratio of 3 or parts fat to 1 part carbohydrate protein will achieve ketosis without producing notable catabolic effects.111 Although success was achieved immediately in various pediatric and adult seizure disorders, use of the diet declined as anticonvulsant medications were developed. However, a resurgence of interest and use occurred in the early 1990s with the recognition that ketosis could provide substantial seizure relief when AEDs failed and without the cognitive toxicity that occasionally accompanies medication.112 In addition to the standard ketogenic diet that is enriched with fat-containing foods, a medium-chain triglyceride diet was developed that was more palatable but commonly accompanied by diarrhea. Use of the ketogenic diet has become associated with the most refractory seizure types of childhood such as atonic, tonic, atypical absence, and myoclonic semiologies. This perception probably is due to the greater efficacy and use of the earlier AEDs for partial seizure types. Although the relief of atonic and atypical absence seizures is frequently the goal of choosing the ketogenic diet, recent studies indicate that the ketogenic diet is effective across a broad range of partial and generalized seizure types.113-115 The standard method of diet initiation includes hospitalization for 5 to 7 days and starvation until ketosis is achieved. Bilizers lithium or valproate which may also each be used alone as first-line treatments ; . Research indicates that an intramuscular injection of olanzapine helps treat agitation in schizophrenia, bipolar disorder, and dementia. Additionally, at lower doses it may manage agitation and psychotic features associated with dementia in the elderly. Olanzapine can cause some sedation, anticholinergic effects, and can cause weight gain. It has been associated with only a small or temporary rise in prolactin levels. Like clozapine, olanzapine blocks many receptors in the brain in addition to dopamine and serotonin. Quetiapine Seroquel ; also blocks a variety of receptors but is more selective for serotonin receptors than dopamine receptors. It is more selective for receptors in the part of the brain responsible for psychotic symptoms and has minimal effect on muscle tone. Quetiapine is effective against psychotic, manic and depressive symptoms. Quetiapine appears to have a low risk of EPS and a low risk of elevating prolactin levels. Side effects may include dizziness, postural hypotension, dry mouth and dyspepsia. The incidence of somnolence drowsiness ; in clinical trials was 18% versus 11% for placebo. Somnolence may actually help elderly patients with dementia who exhibit disturbed sleepwake cycles or evening agitation. What is a "mixed" state? A mixed state is when symptoms of mania and depression occur at the same time. During a mixed state depressed mood accompanies manic activation. What is rapid cycling? Sometimes individuals may experience an increased frequency of episodes. When four or more episodes of illness occur within a 12-month period, the individual is said to have bipolar disorder with rapid cycling. Rapid cycling is more common in women. What are the causes of bipolar disorder? While the exact cause of bipolar disorder is not known, most scientists believe that bipolar disorder is likely caused by multiple factors that interact with each other to produce a chemical imbalance affecting certain parts of the brain. Bipolar disorder often runs in families, and studies suggest a genetic component to the illness. A stressful environment or negative life events may interact with an underlying genetic or biological vulnerability to produce the disorder. There are other possible "triggers" of bipolar episodes: the treatment of depression with an antidepressant medication may trigger a switch into mania, sleep deprivation may trigger mania, or hypothyroidism may produce depression or mood instability. It is important to note that bipolar episodes can and often do occur without any obvious trigger. How is bipolar disorder treated? While there is no cure for bipolar disorder, it is a treatable and manageable illness. After an accurate diagnosis, most people can achieve an optimal level of wellness. Medication is an essential element of successful treatment for people with bipolar disorder. In addition, psychosocial therapies including cognitive-behavioral therapy, interpersonal therapy, family therapy, and psychoeducation are important to help people understand the illness and to internalize skills to cope with the stresses that can trigger episodes. First, operationally, we had a solid third quarter. We are reconfirming our revenue and adjusted diluted earnings per share guidance for 2007 and 2008. Indeed, we are raising the lower end of the ranges we previously provided for revenues and adjusted diluted EPS for 2007. I think it's worth noting that we are on track this year to achieve roughly the same revenues as last year and better adjusted income than last year, despite the loss of exclusivity on two major drugs, Norvasc and Zoloft, and despite the intense challenges facing our largest product. This is due to the strong performance of most of our products, substantial progress in our cost reduction efforts, and the benefits of favorable foreign exchange. Our operational performance, in my view, reflects our insistence on a higher level of focus and accountability within the Company, the advantages of our broadly diversified portfolio, and the benefits of our continued actions to improve productivity. These attributes are an important part of the solid foundation that we are building for the future. With the obvious exception of Exubera, our new products performed very well this quarter. Lyrica grew 37% to 5 million compared to the same period last year, and it has delivered .3 billion in revenues year to date. In June, the FDA granted accelerated approval to Lyrica for the treatment of fibromyalgia, which more than doubles the potential number of U.S. patients who could benefit from this medicine. We were in the field in record time to take advantage of that opportunity. Chantix, our treatment to aid smoking cessation, continues its strong performance, with 1 million in revenues for the quarter and more than 0 million in revenues year to date. Since the August 2006 launch in the U.S., more than 3.5 million smokers have been prescribed Chantix, and we just launched a branded advertising campaign to build on this momentum. When you consider the many countries in Europe and Asia where smoking is still very prevalent, the global opportunities for Chantix are significant. For the year to date, Celebrex is up 10% worldwide and 7% in a U.S. market that is basically flat. Geodon is growing at a rate of two times the market for atypical antipsychotics. And during the quarter, we launched a new Viagra advertising campaign that has generated a lot of interest and positive feedback from the field. We are also continuing to bring strong and creative support to Lipitor, which is facing a commercial assault unprecedented in our industry's history. With new indications and data, advertising and field force support, and contracting strategies to optimize Tier 2 access, we are fighting back hard against branded and generic competition, focusing on both new patients and the switch market. In the face of these challenges, Lipitor revenues in the quarter were .2 billion as compared to .3 billion in the third quarter last year. Now I would like to address a second key aspect of the quarter, our decision to stop further investment in Exubera. This decision reflects our strict adherence to three fundamental principles which will continue to guide us going forward -- we will be realistic; we will listen to our customers; and we will be very disciplined in how we evaluate both internal and external investments so that we can put our capital to work in support of our best opportunities to create shareholder value. Pfizer began the development program for Exubera in collaboration with Nektar Therapeutics over a decade ago. We launched it in July 2006 and made a major effort to make it successful. But despite the best efforts of our sales, marketing and manufacturing colleagues, the product has simply not gained the acceptance of patients and physicians. Accordingly, we will over the next three months support doctors in transitioning their patients to other medicines, and we will cease further investments in the development of a second-generation device. We will redeploy those colleagues in sales, marketing, medical and R&D who have dedicated themselves to this product to activities that are projected to generate higher returns, including important late-stage development projects, as well as stepped-up field force support for our in-line products. We will phase out those manufacturing activities that have been dedicated to Exubera. There are fewer opportunities for redeployment in manufacturing, and we will explore alternatives for sites and employees in consultation with works councils and other appropriate bodies. And we will, of course, carefully evaluate what happened here in order to ensure that we apply all the lessons learned to future product development and marketing and precose.

Ziprasidone Geodon ; is the fifth and newest of the atypical antipsychotics, approved in 2001. It is chemically classified as a benzothiazolylpiperazine. Its pharmacologic activity is comparable to that of quetiapine. It has antagonist activity at several serotonin-receptor subtypes, including 5-HT2A 2C, 5-HT1D, and the D2 dopamine receptors. It has agonist activity at the 5-HT1A serotonin receptors. It has also been shown to cause what is considered to be a mild ECG change--slight prolongation of the QT interval. It is recommended that ziprasidone be taken with food to enhance its absorption.
