NPWT was continued until adequate granulation tissue formed and the patient returned to the operating room on 6 2 for re-debridement and application of an autologous STSG. In conjunction with the STSG, a VAC device was reapplied and set to 75mm Hg of continuous negative pressure to facilitate incorporation of the skin graft on the surgical site. The patient returned to the Wound Healing Center at University Hospital and Medical Center for follow-up care. It was noted that a small portion of the graft had failed and a bio-engineered skin substitute was applied to complete the healing process. The patient was fitted for custom foot gear. He remains healed without complications for the past 14 months. Results: NPWT VAC therapy developed adequate granulation tissue and assisted in the incorporation of an autologous STSG on an irregularly shaped wound bed. Significance of Findings: NPWT may induce the formation of granulation tissue and aid in the incorporation of an autologous STSG on an irregular surface to prevent a more proximal pedal amputation and limb loss. The importance of tissue preservation is paramount to achieving a functional weight bearing surface.
